Patient Referral Process

1. Identify the patient and his/her complaints.
2. Call SomnoDiagnostics, Inc. (501) 321-0547 or toll free (877) 878-6267.
3. Provide SomnoDiagnostics, Inc. with patient and insurance information.
4. Schedule a date and time for the sleep study with SomnoDiagnostics, Inc.
5. SomnoDiagnostics, Inc. will verify insurance benefits and eligibility. SomnoDiagnostics, Inc. will notify the nurse if any referrals or pre-authorizations are required.
6. Fax the Sleep Study Prescription Form to SomnoDiagnostics, Inc. at (501) 321-0386.
7. Anticipate the sleep study interpretation via fax within 72 business hours after study.
